Our health care model includes you, your family, a primary care physician, hospitalist, case manager and social worker and results in a personalized care plan that focuses on your lifestyle and unique medical needs.
Origin
In 1987, Dr. Nayana Vyas founded the first Family Physicians Group medical office in Orlando, Florida. Her vision: to create a healthcare system that ensures that a patient's care and treatment is planned and followed through by a team of experts consisting of a primary care physician, hospitalist, case manager, social worker and most importantly, the patient and his/her family. With over 23 years of experience specializing in family practice and geriatric care, Dr. Vyas has established an organization based on the fundamentals of serving our patients with professionalism and care.
Physicians
Our physicians are dedicated to providing you quality care, preventative services and the most advanced medical treatments. Working as a team, with you and your family, your FPG primary care provider and specialists guide you through your personalized health care plan and offer their support 24 hours a day, 7 days a week.
Case Managers
Our case managers are licensed, registered nurses with extensive health care experience. Their primary goals are to provide you a proactive care plan and the comfort and support you need during each visit to Family Physicians Group.
Health Coaches
FPG’s experienced health coaches help you put your wellness plan into action. Focusing on nutrition and fitness, they provide you the guidance and tools to improve your health and enhance your life.
