Essential duties and responsibilities include the following (other duties may be assigned): (1) Coordinate the care of designated plan members by communicating with the member, family and providers to facilitate the development and implementation of a plan of care that will utilize the most cost effective providers; (2) review and approve referrals for medical services that require pre-authorization; (3) negotiate reimbursement amounts with out-of-network providers when in-network providers are not available; (4) consult with Medical Officers or his designated to review hospitalized members and cases that fail to meet criteria for approval; (5) provides nursing/medical instructions, directions and education to stabilize and maintain health; (6) document and report results of case management interventions; (7) coordinates resources and expertise to improve members health outcomes; (8) serve as a resource to non-licensed staff in the department as well as other departments; (9) provide training to network providers as needed to support the Provider Operations Department; (10) attend continuing education seminars and in-service presentations to keep abreast of current developments; (11) maintain high degree of confidentiality on all patient information.

Three years minimum clinical experience. Two years minimum case management experience (in a managed care environment). RN or LPN registered in Florida. (16) Actively participate in daily coordination of patient care, scheduling, and documentation with designated team members. (17) Participation in marketing events to promote Family Physicians Group. (18) Accurate and timely completion of all documentation. (19) Maintain productivity at acceptable levels as determined by the annual budgets. (20) Assists in evaluating physician candidates as necessary. (21) Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops. (22) Assists with the training of new physicians. (23) Cooperates with co-workers including assisting with orientation and providing peer support. (24) Performs job duties within compliance with Federal, State and applicable Accrediting Body Rules and Regulations and Company and divisional policies and procedures. (25) Maintains confidentiality of patient information with State and Federal law. (26) Follows the Code of Ethics and Business Conduct. (27) Attends annual Compliance education. (28) Reports known or suspected Compliance concerns to Director of Operations. (29) Fairly evaluate employees and independent contractors on Compliance functions. (30) Participate openly and on a timely basis with requests for information or investigations.

Applicants must adhere to the following requirements: (1) Medical degree (MD or DO) and at least three years of medical training (Internal Medicine or Family Practice). (2) Must possess an active, clear physician license in the state of Florida. (3) Minimum of three (3) years of experience as a practicing physician, preferably in Internal Medicine/Family practice and as a Medicare provider. (4) Must be able to work well independently. (5) Board Certified. (6) Practice experience and/or knowledge in providing patient care in the home and senior living facility environment is optimal. (7) Must not be sanctioned by the OIG.

Hospitalists represent one of the most rapidly growing forms of medical practice in the US. Currently a large proportion of hospitalists are recently-graduated residents, who continue familiar duties for a few years. As residency programs are encouraged to limit inpatient duty hours and provide more outpatient education, this pattern may shift. If this specialty evolves as emergency and intensive care medicine did, it will become a formal specialty with its own residencies and board certification within a decade or two. A few distinct residency and fellowship training programs are currently operating at major universities.

Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. Internal Medicine Board Certified. Must possess and active,clear physician license in the state of Florida. Bilingual preferred.
