Transportation

Non-emergency round-trip services are offered free of charge to patients enrolled in a Medicare Advantage Plan.  Certain restrictions apply.  Complete and submit applications at our office and receive transportation in as soon as three weeks.

Service
Family Physicians Group is now offering free rides to qualifying members of our offices in Altamonte Springs, Apopka, Azalea Park, Buena Ventura Lakes, Casselberry, Conway, Edgewood, Hunter's Creek, Kissimmee, Longwood, Metro West, Orlando, Rosemont, Union Park, Waterford Lakes, Winter Park and Winter Springs.  Call to inquire about transportation services in Lake County.

Enrollment*
  1. Find out if your FPG office participates and ask for days and hours of operation.
  2. Participate in or be a member of a Medicare Advantage Plan that FPG accepts.
  3. Confirm that you reside less than ten miles away from an FPG office.
  4. Complete and submit the necessary applications and other paperwork in our office (approval may take up to three weeks).

*Certain limitations apply to patients requiring special transportation needs.
