NCQA Disease Management Accredited (DM)
Earning a full accreditation from the National Committee of Quality Assurance (NCQA) for Disease Management like Family Physicians Group has, assures patients that we have exceeded the standards of performance measures for evidenced-based information, patient safety, customer service and efficient care.
NCQA Diabetes Recognition Program (DRP)
The National Committee for Quality Assurance (NCQA) has verified that Family Physicians Group doctors use evidence-based measures to provide the highest level of diabetes care to their patients. The measures met by FPG to achieve this recognition include: blood pressure, LDL levels, HgBA1c levels, ophthalmologic exams, podiatry exams and nephropathy assessment.
NCQA Heart/Stroke Recognition Program (HSRP)
Family Physicians Group provides excellent care to persons with cardiovascular diseases and persons who have had a stroke. This NCQA program is reserved for managed care organizations like ours who have met key quality measures of care like blood pressure control, complete lipid profile, cholesterol control, smoking status, smoking cessation advice/treatment and the use of aspirin and other antithrombotic drugs.
